Voriconazole can be taken orally or intravenously.

For the oral medicine, in the initial 24 hours, the loading dose should be 400 mg every 12 hours (adults 40 kg and above) for maintenance dose after the first 24 hours,  200 mg twice daily

For intravenous, the dose is 6 mg/kg every 12 hours in the loading phase and 4 mg/kg twice daily in the maintenance phase.

The adverse effects of Voriconazole
